Retreat Participation Agreement

By participating in the Choose Life A&V wellness retreat, you confirm the following:

1. Voluntary Participation — You are voluntarily choosing to attend the Choose Life A&V wellness retreat.
2. Medical Clearance — You have consulted with your healthcare provider prior to participation and have received medical clearance to attend.
3. Program Guidelines — You agree to follow all instructions and guidelines provided by the Choose Life A&V team throughout the retreat.
4. Liability Waiver — You understand that Choose Life A&V, its founders, and staff are not liable for any outcomes that may arise from your voluntary participation in the retreat.
5. Confidentiality — You agree to respect the privacy and confidentiality of all other participants during and after the retreat.
6. Full Agreement — You confirm that you have read this agreement in full and agree to all of its terms.

Prior review of the Retreat Participation Agreement is a mandatory condition of participation in the retreat.
