Health & Liability Waiver By completing your booking, you confirm that you are voluntarily participating in the Choose Life A&V wellness retreat program, which includes physical activities such as massage, exercises, wellness consultations, dietary recommendations and daily routine guidance. 
You acknowledge that you participate at your own risk and that Choose Life A&V, its owners, and staff are not liable for any health outcomes, injuries, or other consequences arising from your participation. We recommend consulting with your healthcare provider prior to joining the program if you have any medical concerns.
